OCC’s Policies and Procedures
For your easy reference, here are important sections of the Member Expectations form you signed when you became a member.
Communication
Emergencies
If you are experiencing a life-threatening emergency, please call 911. Other urgent medical concerns should be addressed with your Primary Care Provider prior to communicating these concerns to the Origins Collaborative Care team. The patient portal is not an appropriate way to communicate urgent medical concerns.
Patient Portal
All health-related communication (outside of appointments) will be done through the HIPAA-compliant patient portal from Cerbo. When you become a Member you will receive an email with information to create a Cerbo account. Portal messages are for succinct inquiries or updates (a few sentences). If a question sent through the portal requires an extensive or detailed response, it will be saved and answered in that week’s Open Office Hours. You will receive a response indicating such.
Phone
You may contact our office via phone at 413-200-9897 Monday-Thursday 8am-4pm. Outside of business hours, you may leave a voicemail which will be checked on the next business day.
Providers at the practice do not use email to communicate with Members because it is not HIPAA-compliant, and therefore it violates our principles to keep your information as safe as possible. You may contact us at office@originscollaborativecare.com for any administrative or logistical questions. Please do not use email for any health-related communication. The practice will not respond to emails or other messages (besides portal) that contain sensitive health information.
Business Hours
Our office is open Monday-Thursday 8am to 4pm. Any alteration in these hours, or holiday closures will be posted on the website here.
Appointment Cancellation and Reschedule Policy
Membership Package Appointments
All membership appointments must be used within 16 months of the OCC Team Intake appointment.
Reschedule: One appointment may be rescheduled within 7 days of the appointment date without incurring a fee. Any additional rescheduled appointments less than a week ahead will be charged a fee of half the value of the visit as defined by the A-La-Carte pricing.
Cancellation: There will be no refunds for canceled membership appointments.
A-La-Carte Appointments
Reschedule: If you reschedule an appointment within 14 days of the appointment date, you will be charged half the cost of the individual appointment.
Cancellation: You may cancel up to 14 days in advance to receive a full refund of the cost of the individual appointment less a $15 processing fee. If you cancel an appointment within 14 days of the appointment date, you forfeit the entire cost of the individual appointment.
Responsibility to Maintain Separate Primary Care Provider
Origins Collaborative Care providers may consult with, but do not replace, care currently provided to you by other providers, such as primary care, internist, gynecologist, cardiologist, gastroenterologist, pediatrician (in the case of children), oncologist or other specialty care providers. Jessica Jimison does not admit patients to the hospital or treat hospitalized patients.
Jessica Jimison does not function as a primary care provider. Rather, Origins Collaborative Care acts as an extension of your medical team, working on root cause resolution. As a condition of receiving services from Origins Collaborative Care PLLC, you must maintain a relationship with an outside provider to act as a primary care provider and to provide emergency and urgent care.
If you encounter a medical emergency and are not able to obtain care from your primary care provider, you are advised to contact 911 or report to a hospital emergency department as appropriate.
