A Discovery Call is your opportunity to connect with
Co-Founders Jessica and Micaela.
Prior to the Discovery Call, we ask you to:
Fill out three short Discovery Questionnaires, which we review closely (see below for where to fill them out!)
Watch Anatomy of a Chronic Illness: The Biology of How We Get Sick, a livestream in which we explain the biology of how Autonomic Nervous System dysregulation shifts the body away from healing and repair and toward increasing symptoms and illness.
In the Discovery Call, we invite you to elaborate on what you shared in your Discovery Questionnaire responses so that we can get an initial sense of the state of your autonomic nervous system, your immune dominance patterns, your mitochondrial health, and your digestive system. Then we will discuss with you how we see your story within the context of Origins Collaborative Care and will suggest next steps for you.
We will not sell or share your information with any parties or entities.
Step 1: Register on Our System
To sign up and get access, we ask you to register yourself as a Prospective Patient on our system. Don’t worry - you are not committing yourself to anything! Getting you in our system allows us to schedule you for the Discovery Call and get you access to the three short Discovery Questionnaires.
Click on “New Patient Registration” (Again, don’t worry! You are signing up as a 'Prospective Patient.’)

Before signing up, please watch Anatomy of a Chronic Illness: The Biology of How We Get Sick and review our Membership page for details on how we offer our care, including membership pricing.
Step 2: Verify Your Email
Once you enter your information to register, you will then need to verify your email. Simply go to your email inbox and click on the email from Origins Collaborative Care PLLC called “Please confirm your registration.”
Step 3: Complete Your Discovery Questionnaires
After you confirm your email, you will see the screen below and are registered for your Discovery Call. The next step is to complete your Discovery Questionnaires.
Step 4: Call our office to schedule your Discovery Call
Once your questionnaires are complete, please send a secure message through this portal to our Member Liaison, Christene DeJong. If you prefer, you can call and leave a message at (413) 200-9897. We look forward to connecting with you soon!
