To become a Member at Origins Collaborative Care PLLC, we need your medical records released and sent to us from your PCP and any specialists you have seen related to the issues we'll be addressing.

Please fill out one record release form for each office, requesting applicable office notes, scan/imaging reports, and labs for at least the last three years.

Each office should follow the instructions on the form and send your records electronically via our Fax #: 413-417-2547

If you need your OCC records sent to your PCP or a specialist, please send a message via the Cerbo Patient Portal requesting our Medical Records Release Form. You will receive a HIPAA-compliant electronic form to fill out.

If you are a current patient of Atkinson Family Practice, please complete this release to authorize AFP to send us your records.
